Membership Application
Note: CCHS is not currently accepting
PLG membership applications.
In order to be eligible for PLG membership, we request that each president
submit:
- a personal statement that addresses one or both of the following questions:
- What have you done to prevent high-risk alcohol use on campus? Relate
this work to the 13 recommendations developed
by the PLG.
- What do you plan to do in future with respect to alcohol and other drug
prevention?
- a copy of the institution’s most recent biennial
review report, as required by the Drug-Free Schools and Campuses Regulation
(34 CFR Part 86)
- two letters of support: one from a member within your institution, one
from a member of the community (state or local)
- a completed PLG membership application form
(Microsoft Word)
- any other materials that can demonstrate what you have done in alcohol
and other drug prevention (e.g., media coverage of programs, policies, and
proposals)
- Once selected, PLG members will be asked to sign an agreement to participate
in the Group’s activities as a statement of commitment.
