The following is a targeted and very short list of resources and tools specifically focused on advance directives. At the end we direct you to one US-based physician-run organization that provides training in communication skills. Tools from Respecting Your Choices Respecting Your Choices Training Manual (1994,1996) edited by Bernard Hammes, with contributions from Bernard Hammes, PhD, Rev. Carol Garman, Nickijo Hager, Elaine Colvin, Dan Felten and Gale Kreibich and published by Lutheran Hospital- LaCrosse.
Resources and Tools Available on Other Websites: A number of other websites offer excellent resources related to advance directives on-line. Most of these organizations encourage the use of their materials for personal use, but do not allow reproduction for commercial use without express permission.
Miscellaneous Resource on Patient-Physician Communication Training The American Academy on Physician and Patient (AAPP) is committed to the improvement of communication, the doctor-patient relationship, and the practice of medicine through the promotion of teaching and research in medical interviewing and related clinical skills.
